Question:

Can you tell me more about when the Z-code is not required?

Answer:

Z – as in zebra – Z-codes are unique identifiers that capture specific information and reports to a Medicare payer what laboratory did the testing and exactly which test was performed. The Z-code is not required for hospitals in non-MolDX states, they are not required for inpatient claims and are not required for payers other than Medicare – but – you may see the MGD Medicare or Medicare Advantage payers request a Z-code be added to the claim when billing for outpatient procedures.
