What code should be used on claims for CLIA-waived tests?

Answer:

For claims with dates of service (DOS) on and after January 1, 2016, the following code should be reported instead:

G0477QW Drug tests(s), presumptive, any number of drug classes; any number of devices or procedures, (e.g., immunoassay) capable of being read by direct optical observation only (e.g., dipsticks, cups, cards, cartridges), includes sample validation when performed, per date of service

For claims with DOS up until December 31, 2015, the following code is reported: 

G0434QW Drug screen, other than chromatographic; any number of drug classes, by CLIA waived test or moderate complexity test, per patient encounter
