Question:

Can a diagnosis code be reported more than once?

Answer:

“Each unique ICD-10-CM diagnosis code may be reported only once for an encounter,” according to item 12 of the general coding guidelines in the FY 2017 ICD-10-CM Official Guidelines for Coding and Reporting. It also states, “This applies to bilateral conditions when there are no distinct codes identifying laterality or two different conditions classified to the same ICD-10-CM diagnosis code.”
