Question:

What code would be reported for the following scenario?

A patient came to the ER with an acute myocardial infarction (AMI) and was rushed to the cath lab within 90 minutes. The cardiologist performed an angioplasty of the LAD including aspirating thrombus.  The revascularization is a success.

Answer:

You would report 92941 LD (percutaneous transluminal revascularization of acute total/subtotal occlusion during an AMI, coronary artery or coronary artery bypass graft, any combination of intracoronary stent, atherectomy and angioplasty including aspiration thrombectomy when performed, single vessel). 

The above scenario meets the code description since the patient had an intervention within 90 minutes or thereof. The code specifies “any combination,” and angioplasty fulfills that requirement. The aspiration thrombus removal is inclusive in 92941 and not separately reportable.
