Question:

What codes should be reported for coronary artery bypass procedures using venous grafts?

Answer:

The National Correct Coding Initiative Policy Manual for Medicare Services includes directions to not report 37700–37735 with 33510–33523. Under the bypass section, the CPT directions state that the saphenous vein harvesting is inclusive in the bypass codes, whether just venous bypass, or venous arterial bypass procedures are performed. No codes are available that describe saphenous vein harvesting, and codes 37700–37735 (ligation and stripping) were being reported in error.  If an upper extremity vein is harvested (+35500) or femoral-popliteal vein is harvested (+35572), as noted in the parenthetical, report these codes as appropriate in addition to the bypass procedure (33510–33523).
