Question:

Can a code for a left heart cath be assigned when the physician does not document crossing the aortic valve or does not document any reference to the left ventricle or aortic valve, but only gives LV pressures? On this example the department wants to charge LHC/RHC with coronary arteries.

Answer:

With the documentation provided, even though there is nothing in the technique supporting a left heart catheterization, since LV pressures were taken and documented, code 93460 (right and left heart catheterization with coronary angiogram) would be appropriate.
