Question:

What is the appropriate code for instant wave-free ratio performed during a cardiac catheterization and PCI?

Answer:

According to CPT Assistant, May 2015, instant wave-free ratio is similar enough to fractional flow reserve (FFR), that the following codes would apply. Since these codes include pharmacological induction of stress, in cases where no pharmacologically induced stress was done, modifier 52 (reduced services) would need to be appended.
