What code would be assigned when only one lead on a dual-chamber pacemaker is replaced?

Answer:

The removal/extraction of a lead depends on the type of system, but the placement depends on how many leads, not the type of system. So, if one lead of a dual system is removed and replaced, you would assign the following codes:

33235 Removal of transvenous pacemaker electrode(s); dual lead system
33216 Insertion of a single transvenous electrode, permanent pacemaker or implantable defibrillator

If the lead on a single-lead system is removed and replaced, you would assign the following codes:

33234 Removal of transvenous pacemaker electrode(s); single lead system, atrial or ventricular
33216 Insertion of a single transvenous electrode, permanent pacemaker or implantable defibrillator
