Answer:

In addition to reporting codes C8921–C8930, report the following codes based on the amount of contrast milliliters (ml) used: Q9950 (Injection sulfur hexafluoride lipid microspheres [Lumason®]); Q9955 (Injection perflexane lipid microspheres, per ml [Imagent®]); Q9956 (Injection octafluoropropane microspheres, per ml [Definity®]); Q9957 (Injection perflutren lipid microspheres, per ml [Optison™]). For OPPS billing, only Level II HCPCS code Q9950 is subject to pass-through status.
