One of our patients had coronary stents inserted last week. Yesterday he had a myocardial infarction (MI). The doctor ballooned one of the stents to open it, but no new stent was placed. Can we still assign 92941 since it says “combination”? The hospital wants to code for a new stent placement.

Answer:

You can’t code for a stent placement if no stent was placed at this session. If the angioplasty was performed in the culprit vessel during the acute phase of an MI, then 92941 should be assigned.

Although the code description says “combination of angioplasty, stent or atherectomy,” it is not necessary to use two or more types of treatment. In CPT Assistant January 2014, the AMA clarified that 92941 can be assigned if one or more therapies were used.
