Question

Where can we find the documentation that says we can’t use code 71090 (fluoroscopy) for pacemaker insertions? Can that code still be charged for pacemaker removal?

Answer

The American Medical Association (AMA) deleted code 71090 effective January 1, 2013, so it can no longer be assigned for fluoroscopy. When the AMA revised the pacemaker codes in 2012, they were valued to include imaging guidance.

In the CPT book, the guidelines at the pacemaker/ICD section include the following statement:  “Radiological supervision and interpretation related to the pacemaker or pacing cardioverter-defibrillator procedure is included in 33206–33249. To report fluoroscopic guidance for diagnostic lead evaluation without lead insertion, replacement or revision procedures, use 76000.”
