We implanted an internal looping recorder (ILR) in a patient, and now we need to do a pocket revision. Can we use the revision code (33222) for pacemaker for an ILR, or do we need to use an unlisted code?

Answer

Code 33222 would not be appropriate for a revision, even if this was a pacemaker and not an ILR. The AMA revised codes 33222 and 33224 in 2014, and now, they only apply to relocation.   

If your pocket revision is being performed because of hematoma or infection, see incision and drainage (I & D) codes such as 10140, 10180, or debridement codes such as 11042–11047 as appropriate.
