We have begun tomosynthesis-guided breast biopsy/localization. There is no mention of stereotactic or mammographic guidance. Do we report this like we report stereotactic biopsies?

Unfortunately, there is no code for tomosynthesis-only guided biopsy or localization. You would report unlisted code 19499 to include both the guidance and the biopsy and/or localization. If you used a combination of stereotactic and tomosynthesis guidance, you would report code 19081 for the biopsy or 19283 for the localization.

19081 Biopsy, breast, with placement of breast localization device(s) (e.g., clip, metallic pellet), when performed, and imaging of the biopsy specimen, when performed, percutaneous; first lesion, including stereotactic guidance.
19283 Placement of breast localization device(s) (e.g., clip, metallic pellet, wire/needle, radioactive seeds), percutaneous; first lesion, including stereotactic guidance
