If a patient comes in for a two-day whole-body tumor scan with SPECT, we bill 78804 and 78803. If the ordering physician wants SPECT of more than one area, can we charge 78803 more than once?

Answer

You can bill 78803 only once per session. The Society of Nuclear Medicine and Molecular Imaging has said that SPECT performed on separate days may be separately coded. However, Clinical Examples in Radiology, co-published by the AMA and ACR has published case studies that indicate SPECT may be coded once even when performed on separate days.
