We had a patient with lesions in the left anterior descending artery (LAD) and one of the diagonal branches. We placed a drug-eluting stent (DES) in the LAD and then performed angioplasty of the diagonal. We assigned the following code for the LAD stent:

C9600 Percutaneous transcatheter placement of drug eluting intracoronary stent(s), with coronary angioplasty when performed; single major coronary artery or branch

Do we assign code 92920 or 92921 for the angioplasty of the diagonal?

Answer

You would report 92921 for the angioplasty of the diagonal branch. Only one primary intervention code may be assigned for a major coronary artery and its branches. Since the diagonal is a branch of the LAD, the additional vessel code would be assigned for the angioplasty.

+92921 each additional branch of a major coronary artery (List separately in addition to code for primary procedure)
